Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health care needs through communication and available resources to promote quality cost-effective outcomes. It is grounded in advocacy, communication, and resource management. 

Our team works closely with the patient, family, health care providers, TPA, stop-loss carrier, consultants, Clients, and community. 

Nurse case managers utilize our proprietary Community Resource Guide that provides members with free health and financial resources to support the prescribed medical treatment plan. 

Our goal is to ensure that identified members receive the appropriate services and support to meet their health care needs resulting in a reduction of health care expenditures.
